but if you need to write a check, please make it payable to GWA.

The shirts are 100% cotton red or black
and will be imprinted in white,red or black depending on your
child's team
color.
The shirts come in sizes youth
small to adult XL.
Adult XXL and XXXL
are also available for a cost of $9.00.
Please send in money
along with the bottom of this form to your child's
teacher by Monday, April 11, 2016.
Please order
and pay for each of your children’s shirts through their own teacher. This
helps both Mrs. Scott and the teachers keep the
